Event/Room Request Form

This form will be submitted to the LifePoint Church Facilities Manager. Requests will be considered in the order they are received, coordinated according to LifePoint’s church event calendar, and approved by the Facilities Manager. Any requests that include set up and take down assistance must provide an Event Setup Diagram provided to the church. All users of the facilities, both members and non-members, must agree to and conform to the LifePoint Building Use Policies document. By submitting this form you agree to all terms and conditions according to the LifePoint Church Bangkok Facility Use Policy. You are also agreeing to pay any fees and/or charges that may be required by the church.
